Requirements and Fees

      Mail in registration forms must be post marked by August 16th.         

Mail In Registration:

(1) Registration form must be signed by the player's parent or legal guardian.  (2) Please send check or money order, registration form and (3) a copy of your child's birth certificate when registering by mail.
